Booster sessions are intended to provide information on the transition from school to adult life for youth with disabilities. Families, youth and individuals with disabilities, and professionals are invited to attend these sessions at no cost.

Attendees will learn:

• The keys to achieving a successful medical transition
• Skills that can help youth and families plan and be prepared
• About local transition resources and new initiatives in the area
