MCBDD is partnering with Discount Drug Mart to provide Flu vaccines (ages 19 and up) and COVID vaccine and boosters (ages 12 and up-Pfizer only). See attached flyer and forms for more information.

Clinic Registration – REQUIRED

When attending the clinic you MUST:

1. Review the Flu Vaccine Information Sheet

2. Print, complete and bring a consent form for each person receiving a vaccine or booster.

Consent Form – REQUIRED

3. Bring your insurance card or information.

4. If receiving a Covid booster, bring your current Covid Vaccination Card – participant must have the primary series completed and must be at least 2 months since last COVID shot to receive the new booster.
