The Medina County Board of DD is partnering with Discount Drug Mart and the Autism Society of Greater Akron for a Flu Vaccine Clinic for community members ages 19+.

Clinic Registration – REQUIRED
When attending the clinic you MUST:
1. Review the Flu Vaccine Information Sheet
2. Print, complete and bring a consent form for each person receiving a vaccine.
3. Bring your insurance card or information.
