Transition Services



Until students are ready to graduate, parents have had years of being involved in Individual Educational Plans (IEPs) and meetings. As a child leaves school and moves on to the more adult world, there may have concerns about the issues that you and your family may face. Transition is about creating a path to an adult life that includes meaningful connections to the community through work, life and social opportunities. Our Transition Services are here to help guide your family through this time of change and growth.
 

Our Transition Services Specialist helps coordinate activities that promote:

  • continuing education
  • vocational education
  • integrated employment (including supported employment)
  • adult services
  • independent living
  • daily living skills
  • community participation



Transition Timeline
We know the earlier we start working with students to develop future goals, the more successful we can be in finding all the opportunities that await them. In fact, at age 14, state law requires that students begin receiving Transition Services and that those services are included in the student’s Individual Educational Plans (IEP).
