Provider Portal


Providers may be either Independent Providers (self-employed) or Agency Providers. Becoming an Individual Certified Provider can be a rewarding experience; however, it also requires that you complete several steps in order to meet the certification requirements specified by state rule.  The certification process is completed through the Ohio Department of Disabilities (DODD).

Tips for Completing Your Application:

  • A birth certificate is a required document and will need to be submitted.
  • It is recommended that applicants first gather all documents (except the BCII) prior to completing the application.
  • Once your fee has been paid, you will have 30 days to submit all of your required documentation.

For the sake of the individuals we support, it is important to keep your certification current and to be aware of when your certification expires.  Independent providers renew their certification every three years.  You may contact us if you do not know when your certification expires.

To complete recertification, you will need the following documents:

  • Current report from the Bureau of Criminal Identification and Investigation (BCII)
  • First Aid Certification: Valid American Red Cross or equivalent certification in First Aid
  • CPR Certification: Valid American Red Cross or equivalent certification in CPR
  • Completion of Annual Required Training: Evidence of completion of annual training on MUI and Client Rights. This may include a certificate provided upon completion of all eight hours of Initial Provider training.

The health and safety of people with developmental disabilities is the top priority for the Medina County Board of Developmental Disabilities. Our board members, administration and staff are committed to a community that is healthy and safe for all those with disabilities, and providers are expected to do the same. MCBDD is designated by the state as the reporting agency for incidents involving individuals with developmental disabilities in Medina County.
