MUI reporting and investigations help everyone to identify why an incident occurs and to prevent it from happening again. All MUIs reported to the Medina County Board of Developmental Disabilities are sent to the Ohio Department of Developmental Disabilities.

Who Must Report an MUI and How

All providers who are contracted, certified or licensed to serve persons with developmental disabilities, all Medina County Board of DD staff and anyone paid to provide Medicaid waiver services are required, by law, to report MUIs.

Incidents involving abuse, neglect, death, exploitation, misappropriation, failure to report, and peer-to-peer acts must always be reported within four hours of occurring. All other incidents must be reported by 3:00 pm the next working day.

All providers of service are required to report an MUI by using the Provider Initial MUI Report Form.

MUIs may be reported during working hours (Monday – Friday, from 8:00 am – 4:00 pm), through an email to muireport@mcbdd.org.

Emergency Cell

During all other times (Monday – Friday from 4:00 pm – 8:00 am and weekends), reports are handled through an emergency cell phone at 330-725-7751 option 4.
